It’s Feedback Time! (Participants only)
Subject line: Action Required – It’s feedback time!
Hi [User First Name],
We are excited to be kicking off your feedback process! This email will provide essential information about the process and walk you through your next steps. **Please read this entire email before getting started.**
Why is feedback important?
Feedback is one of the most powerful tools for professional growth, especially from the people who know your work best.
Unlike traditional performance reviews, this feedback isn’t about grading your past but fueling your future. By gathering perspectives from colleagues who work alongside you, the assessment offers a well-rounded view of how your communication and collaboration show up to others and where you can grow next. Your colleagues will score you on a series of specific behaviors within these categories:
– Professionalism & Self-Awareness
– Management & Leadership
– Communication Skills
– Teamwork & Camaraderie
At the end of this process, you’ll receive a report highlighting key strengths and opportunities for growth. It’s designed to help you strengthen relationships, build on your strengths, and take intentional steps in your professional development. Strong working relationships don’t just improve collaboration; they drive better business performance.
You will also complete a 4-minute self-assessment that mirrors the feedback form to see how your perception of your skills and behavior might differ from that of your colleagues.
Who sees my results?
The feedback from your colleagues is anonymous and confidential. No one from your organization has access to that data. When the process concludes, you will be the only one who receives your feedback report, with the following exception: if your report indicates any legal, ethical, or other similar concerns, your organization’s leadership will be notified of those results.
What do I need to do?
First, complete your self-assessment. Second, check to see if anyone requested feedback from you. If so, complete those feedback forms. To access both, click the button below.

It’s Feedback Time! (Raters only)
Subject line: Action Required – It’s feedback time!
Hi [User First Name],
You have been asked to provide feedback to one or more colleagues! This email will provide essential information about the process and walk you through your next steps. **Please read this entire email before getting started.**
## Why is feedback important?
Feedback is one of the most powerful tools for professional growth, and yours plays a key role in that process.
Your input will help your colleague understand how they’re perceived at work, how their communication, collaboration, and day-to-day behaviors show up to others. This process is focused on the recipients’ future development and growth. You’ll be asked to share your perspective on specific behaviors to help them reflect, grow, and strengthen their working relationships. Feedback will be focused on four areas:
– Professionalism & Self-Awareness
– Management & Leadership
– Communication Skills
– Teamwork & Camaraderie
You will be asked to rate the participant on various behaviors related to these areas. Be honest in your feedback! Your responses are entirely anonymous (see below), so please provide candid feedback to assist the participant in their professional development. Each feedback form takes, on average, less than seven minutes to complete.
## What do I need to do?
It’s time to complete some feedback forms! You can access your feedback forms by clicking on the button below.

Declining Feedback
Focus on giving feedback where you have experience with the participant. Please skip any questions that don’t apply to your working relationship. A response of “3” isn’t the same as “N/A” and will affect the overall score. If a statement doesn’t apply, just skip it!
Additionally, if you lack experience with a particular participant, you can decline their feedback form directly on the platform to clear their form from your list and stop receiving reminders.
## Who sees my responses?
The feedback you provide is completely anonymous. Your responses will be compiled with other reviewers’ feedback to generate an anonymous summary report. No one from your organization has access to those results. The person receiving the feedback will be the only person in your organization who gets that report, with the following exception: if your report indicates any legal, ethical, or other similar concerns, your organization’s leadership will be notified of those results.
If you have any technical issues, please contact [Support Email]. We will follow up with reminders and additional information as we progress.

Not Report Eligible
Subject Line: An Update in the Upward Feedback Process
Hi [User First Name],
Thank you for participating in the recent upward feedback process. Your engagement directly supports your team’s growth and helps build an open, honest communication culture.
At this time, you aren’t eligible to receive a feedback report. This may be because you did not complete your self-assessment, an essential component for report generation, or because there wasn’t enough feedback gathered to ensure anonymity for those who shared insights.
Please contact your organization’s upward feedback administrator if you have any questions about this process. If you have any technical issues, please contact [Support Email].
